Does the University of Chicago Hospital use MyChart? ›

Our MyChart patient portal offers you secure online access to manage your health. Using the patient portal you can schedule medical appointments, view test results, request prescription renewals, communicate with your care team, pay your bill, manage care for a loved one (proxy access) and more.

Enter your MyChart activation code exactly as it appears. Your activation code will be printed on your after-visit summary, can be printed for you or sent to the email address we have on file for you, upon your request. You will not need to use this code after you have completed the sign-up process.

Anyone with a MyChart account can download and use the app for free. Go to the App Store (for iOS) or Google Play (for Android) on your mobile device to download the app.

How is MyChart secure? Your healthcare providers take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords.

You can make a written request to either review or obtain a copy of your medical records pursuant to Health and Safety Code sections 123100 through 123149.5. You can view these laws on the California Legislative Information website.

MyChart is EPIC's patient portal. MyChart is a secure means for patients to access parts of their health records through a web browser and cell-phone apps.

Some test results are not released to your MyChart account because of their sensitive nature. Your doctor determines which types of test results are able to be accessed through MyChart and which might require an in person conversation.
